Breathe Clinic works with many major insurance carriers. To request an appointment, you may complete the form below or call us at 951-371-6600 ext. 1. Our scheduling department will contact you to confirm a time that best fits your schedule.

In most cases, your first appointment will be a consultation with a physician. To make sure you receive the highest quality care, it’s important that the physician review your medical history.

Please bring the following items to your first appointment:

• Insurance card
• Insurance authorization (if required)
• A list of your current medications
• A list of your questions
• Pertinent medical reports, letters, x-rays, CDs or files
• SD card from your sleep machine (if applicable)

If you request your medical records be sent from other doctors or medical facilities, please call our office prior to your appointment to make sure we have received them.
